Restorative Neurostimulation for Chronic Low Back Pain Due to Multifidus Muscle Dysfunction

Procedure Description


The ReActiv8® Procedure is a minimally invasive implantable system that delivers restorative neurostimulation to the multifidus muscles, deep spinal stabilizers that often become impaired in patients with chronic low back pain. Unlike treatments that only mask pain, ReActiv8 addresses the underlying cause of mechanical instability by reactivating these muscles through targeted electrical stimulation.

This therapy is FDA-approved for patients with chronic axial low back pain associated with multifidus dysfunction, especially those unresponsive to conservative care such as medications, physical therapy, and injections.


What It Treats


ReActiv8 is designed for patients with:

  • Chronic mechanical low back pain lasting 6 months or longer

  • Evidence of multifidus muscle dysfunction (often confirmed by physical exam or imaging)

  • No indication for surgery, or after failed spine surgery

  • Poor response to traditional therapies, including:

    • Physical therapy

    • Epidural steroid injections

    • Medications

Symptoms commonly include deep, aching back pain, worsening with activity or prolonged posture, and no significant radiating leg pain.


How It Is Performed


  1. You will lie face down under local anesthesia and sedation or general anesthesia, depending on your provider’s recommendation.

  2. Two small incisions (about 1 inch) are made over the lumbar spine, typically at the L3–S1 level.

  3. Using fluoroscopy (X-ray guidance), two electrical leads are placed near the medial branches of the dorsal ramus nerves, which control the multifidus muscles.

  4. Leads are connected to a small pulse generator, implanted under the skin, usually in the flank or upper buttock.

  5. The system is tested and programmed before completing the procedure.

The procedure typically takes 60–90 minutes and is performed in an outpatient or short-stay surgical setting.


Pre-Procedure Instructions


  • Do not eat or drink for at least 6–8 hours before the procedure if sedation is planned.

  • Discuss all current medications with your provider—blood thinners and diabetes medications may require special instructions.

  • Inform your provider if you:

    • Have metal implants or devices

    • Are pregnant or planning pregnancy

    • Have an active infection or recent illness

  • Arrange for transportation on the day of the procedure.

  • Wear loose, comfortable clothing and remove all jewelry prior to arrival.


What to Expect After the Procedure


  • Mild soreness or swelling at the incision site may occur and typically resolves in a few days.

  • The ReActiv8 system is activated a few days after surgery, and your provider will customize the stimulation settings.

  • You’ll use a wireless remote control to begin twice-daily 30-minute sessions, which activate the multifidus muscles.

  • Over time, stimulation helps restore neuromuscular control, reduce pain, and improve spinal stability and function.

  • Many patients experience meaningful improvement by 8–12 weeks, with continued benefit over 6–12 months.

Most patients can resume normal light activities within a week, avoiding strenuous bending, lifting, or twisting for 4–6 weeks.

Call your provider if you experience:

  • Signs of infection at the incision site (redness, swelling, drainage)

  • Device-related discomfort or malfunction

  • New or worsening symptoms
